        To obtain a birth certificate using a major credit card (Visa, MasterCard, American Express or Discover), call (317) 233-2700. You will be asked for the following information:

        • Credit card name, number and expiration date.
        • Type of certificate you are requesting. Full name at birth, date and place of birth.
        • Full name of father and mother's maiden name.
        • Daytime phone number and address where to send the birth certificate.

The specific city or county of death must be provided for searches in this period and the search is limited to one county for each $4.00 search fee paid. For records prior to 1900, write to the Health Officer in the city or county where the event occurred. Fees vary.
