Puerto Rico Vital Records Office Information
Make USPS money order payable to Secretary of the Treasury. Personal checks are NOT accepted. Include a signed letter, a copy of an Identification Card along with the appropriate information for the record being ordered. Copies of earlier records may be obtained by writing to the local Registrar (Registrador Demografico) in the municipality where the event occurred or by writing to the central office for information.
