Make check or money order payable to Vital Records. A $0.50 state charge must be added to each mail or Vitalchek order. Include phone number(s) and SASE in request. All mail orders MUST be accompanied by a photocopy of your driver'slicense or photo ID, regardless of relationship of requestor and/or registrant/decedent. Failure to include same may result in your request being denied.
If the requestor is NOT the registrant, and is NOT immediate family (IN THE SAME HOUSEHOLD), the requestor must also supply a SIGNED statement ofauthorization and a photocopy of BOTH the registrant's and the requestor's ID. This isespecially applicable to cousins, aunts, uncles, nieces, nephews, brothers, sisters.Full eligibility requirements are also posted on the site and should be read BEFORE placing an order.
Fraudulent applications WILL be prosecuted in accordance with Louisianalaw. In the event that fraudulent documents are submitted in a request, ALL parties involved can be prosecuted.

If no record is on file, a search fee equivalent to the cost of one copy of the record will be charged. Please note: You can obtain ONLY birth records over 100 years old and Death records over 50 years at the Louisiana State Archives. All other current vital records are obtained through the Louisiana Department of Health and Hospitals.

Marriage and divorce records are available from the Clerk of the Court in the parish where the license was issued or the divorce was granted. Birth and death certificates are no longer available in the individual Parishes.
